The form and procedure for reporting controlled transactions has been established

On 14 January 2014 the Ministryof Revenues and Duties of Ukraine published the approved* form of the Report onControlled Transactions and the procedure for its preparation (with relevantappendixes). 

The report format is relatively standard, but significant information will beneeded to complete the appendixes.
